Various healthcare providers, including physicians and the following Non-Physician Practitioners (NPPs), can deliver Chronic Care Management (CCM) services:
• Nurse Practitioners (NPs)
• Physician Assistants (PAs)
• Certified Medical Assistant (CMA)

CCM services can be billed at two levels: non-complex and complex. These levels are differentiated based on clinical staff service time, involvement of billing practitioner, and extent of care planning; care is deemed complex if it requires more than 60 mins per month. Only one type of CCM service can be billed per patient per month.
This code is used for the first 20 minutes of care provided in a calendar month by clinical staff and directed by a physician or another qualified healthcare professional.
This code is used for every additional 20-minute increment of service time that exceeds the 20-minute time allotment of CPT® 99490.
This code is used for CCM services that total at least 30 minutes per calendar month and are provided by a physician or another qualified healthcare professional.
This code is used for an additional 30-minute increment of service time that exceeds the 30-minute time allotment of CPT® 99491.
This code is used for complex CCM services that total at least 60 minutes of clinical staff time per calendar month and are directed by a physician or other qualified healthcare professional.
This code is used for an additional 30-minute increment of complex CCM provided in a calendar month.
