Who can prescribe CCM?

Various healthcare providers, including physicians and the following Non-Physician Practitioners (NPPs), can deliver Chronic Care Management (CCM) services:

• Nurse Practitioners (NPs)
• Physician Assistants (PAs)
• Certified Medical Assistant (CMA)

Billable CPT® Codes

CCM services can be billed at two levels: non-complex and complex. These levels are differentiated based on clinical staff service time, involvement of billing practitioner, and extent of care planning; care is deemed complex if it requires more than 60 mins per month. Only one type of CCM service can be billed per patient per month.
